Intake Forms & Waivers

Before your first appointment, you may be asked to complete digital intake forms and sign service agreements. These protect both you and your salon by ensuring everyone is on the same page about the services being provided.

Types of Forms

Service Agreement

The service agreement outlines the terms of grooming services, including:

• Scope of services and what is included in each package.
• Your salon's cancellation and no-show policies.
• Payment terms and when charges are applied.
• How your personal and pet data is stored and used.

Liability Waiver

Grooming involves sharp tools and working with animals who may be stressed. The liability waiver acknowledges that:

• Minor incidents (small nicks, clipper irritation) can occur despite the highest care standards.
• Matted coats may require shaving which can reveal underlying skin conditions.
• Your salon is authorized to seek emergency veterinary care if needed (at the owner's expense).
• You have disclosed any known medical conditions, allergies, or behavioral concerns.

Photo & Video Consent

Some salons request permission to photograph your pet for:

• Grooming records: Before and after photos for your appointment history.
• Social media: Marketing posts featuring your pet (optional — you can decline).

How to Complete Forms

1. You'll be prompted to complete forms during checkout for your first booking, or when the salon updates their agreements.
2. Read each form carefully. Tap to expand sections for full details.
3. Sign electronically by typing your name or drawing your signature.
4. Your signed forms are saved to your account and can be viewed at any time under My Account > Forms & Agreements.

Updating or Revoking Consent

• You can update your photo consent preference at any time from My Account > Privacy.
• Service agreements and liability waivers remain in effect for the duration of your relationship with the salon.
• If the salon updates their agreements, you'll be asked to review and re-sign before your next appointment.

Your rights: You can request a copy of all signed forms at any time by contacting the salon or downloading them from your account.
